Do they not include Adapter to Micro sd card to plugin to p.c. anymore ?

Why is it not included anymore ? They use to include it.

Some still come with an adapter, but they aren't always listed in the details. A lot of computers and card readers can now take the micro SD card directly without an adapter as well

My Android phone says it needs memory card with Fat format.

I am using Lexar 32 gigs. micro sd card.

Do I need to re-format the sd card for it to work ?

Its likely formatted fat32, your phone would have an option to format it to the format it needs if it doesn't work right out of the box
